Membership Plans

Individual Monthly Plan

• $99 per month cost.
• This plan entitles you to four office visits(adjustments) per month. Visits do not roll over month to month.
• Walk-in and/or appointments are welcome.
• Each additional visit is only $25.

Family Monthly Plan

• $89 per month cost for one adult  PLUS $49 per month per each additional adult (20 years old or older). 
• Each additional member age 0 to 19 years old is $29.
• This plan entitles you to four office visits(adjustments) per month per member. Visits do not roll over month to month.
• Walk-in and/or appointments are welcome.
• Each additional visit is only  $25.

Yearly Packages available (use within one year at any time)

• 6 total visits in 12 months    $249 (less than $42 a visit)
• 10 total visits in 12 months  $369 (less than $37 a visit)
• 20 total visits in 12 months  $659 (less than $33 a visit)

Add-ons to each package per visit

Spinal Decompression: $39 

Electric Muscle Stimulation: $15

Ultrasound Therapy: $15

Functional Rehabilitation: $39 

Osaki Massage Chair: $15

Membership and Fees Information

• Discount of 15% off for each plan that is paid upfront for each six month period. 
• New Patient Referral Bonus: get one month free for each referral resulting in a signed commitment agreement. 

*Due to federal guidelines, participants in Medicare and Medicaid replacement plans are not eligible for free or discounted offers.
